1994-P Nickel: Double-Sided Jefferson (Reverse Is 1964 Jefferson)

Hey all, I have a nickel with Jefferson on both sides. The obverse says 1994-P, the reverse says 1964. I can't find anything online that's similar so I'm wondering if it's even real.The wear and tear show it was in circulation for awhile before ending up in my possession. Does anyone have information on this? Photos for reference.
