Can't tell you what caused it, but I think it is a spender.

You might never find out exactly what happened to it but the fact that it could not
have happened when the coin was struck means that is damage of some sort.

Perhaps a bad cleaning a while ago, with crud left behind after a liquid substance evaporated. Might be fun (yes, fun), to give the coin a long soak in 100% acetone.
