This might just be PMD but the area on the reverse looks like the shape of a screw. Opinions?

As noted, this is just damage from contact with a reeded edge coin. You should be able to figure out which coin caused the damage by comparing the edge of a dime or quarter (or ?) to the mark so you can check the spacing.
