What causes this to happen to a coin? I'm not sure if it was an error or if maybe the coin met a toxic substance that scratched one side and not another?

How it happened is anyone's guess, but it is PMD not an error.

Yes, post-strike damage. The coin didn't leave the striking chamber in that condition.

Parking lot coin - somebody got lucky and skidded with a car or something on it gouging the reverse side.
