2012 Washington Quarter - Finish All Distorted

I found this in a coin roll. The obverse and reverse have issues. What causes this to happen?

Lots of things can distort the device elements on a coin, like mechanical damage, or acid, or just an acidic environment.
In your case, I think it was cause by a set is dies that should have been replaced long before this coin was struck.
On it I see extreme Die Deterioration Doubling and Machine Doubling too.
