It looks like a very old coating of lacquer was applied to this coin and now it is coming off - in my books it is now a damaged/altered coin.

Possibly ex-jewelry with the dent in the rim from bezel damage and painted or plated gold for appearance. I think you're confusing the circular damage in the rim with a misaligned die.

From experience, I know that isn't significant enough to get a TPG to put 'MAD' on a slab.

Here is my MAD Morgan, and this is how far a Morgan has to be off to be slabbed as such.

That is damage on the rim.

As for the stuff all over the coin, it could well be lacquer as suggested. I would give the coin a soak in acetone and see if it removes that. It would at least improve its appearance without harming the coin.

And if it is lacquer, that would not damage or alter the coin. It is simply a coating that can be easily removed with nothing done to the coins surface. Once done, correctly, you would never be able to tell it had been lacquered.
