I've read up on MADs and Collar issues, there are many events that can cause anomalies on coins.

Is this a MAD or Collar issue or other?

I think it's from advanced Die Deterioration.

Not Die Deterioration. I think John's theories are the best explanation.

Possible bezel. A clear, end-on pic of the rim might tell us more.

Well, there's Die Deterioration on both sides. The die was worn, when it struck this coin. It could be what John1 said, we'll see.
