Yes that is an interesting one. Here are a couple link to similar game tokens on numista, but one of them has a groove cut on the reverse:

Also, mention of the rotation is made in both entries so perhaps all of them were made that way.

It's a curiously multi-lingual token. "BON POUR UN JEU GRATUIT" is French for "good for one free game". "Firenze" is the name of the city of Florence, in Italian, but the word "Italy" is the English spelling. "QUEIROLO FRANCESCO" is the name of an Italian artist from the 1700s, only backwards (Francesco Queirolo); I would assume this is actually the name of a now-extinct company in Florence that made the tokens, and the QF logo is the company logo. A Google search for ["QUEIROLO FRANCESCO" token] comes up with a bunch of tokens exactly like this, plus a couple of tokens purely in Italian.

I'd agree I don't think the odd die rotation is an error. Private token manufacturers have much less need than government mints to make sure their die alignments are correct, so there isn't necessarily a "normal" or "correct" orientation.
Don't say "infinitely" when you mean "very"; otherwise, you'll have no word left when you want to talk about something really infinite. - C. S. Lewis
