1994 P Washington Quarter/Planchet Error?

Good morning. This coin is definitely under weight but is it due to environmental damage, deliberate caused by someone or could it be something more? There is odd markings on the coin but it could be simply coincidence. Well here's a few images. I know they aren't the greatest but if better ones are needed ill try and get them on here. It weighs 5.14 grams.....

Appears to be post-strike damage--a good degree of circulation wear, plus exposure to environmental agents which resulted in the discoloration and light corrosion of the coin's surfaces. The coin may have spent some time in the ground.

I've never seen one that light from being in the ground. Not to mention its not all "Mushy" and have some edge to its detail. Brown is a typical ground coin. This has a steel look to it WHICH I KNOW IT IS NOT. 5.14 G....

I agree with above and the worn reeded edge is lost likely the coin used to be slot machine coin before ending up in the ground.

Well it is under weight and out of specs, but it could have started a bit light on a thin planchet.
Here is a nice graph for weights on US coins.
