Here are a few more with better lighting.
Sent it to Anacs on the advice from a very knowledgeable local coin person.
I've searched the web and cannot find any other ones

Thanks for sharing. Begs the question: Is it a Mint error or a post-Mint "creation"? The ANACS label description is ambiguous, IMHO.

Slag is impurities in a molten metal that are not removed in processing and carried over to the finished product.

I think "inside blank" is the terminology that's a bit confusing.
