Looks like a 20 santimu 1922 regular version km 5. Could be a pattern strike like known for the 10 santimu 1922 known. Cannot see whether this coin is silver or aluminium-bronze, which are patternstrikes in Latvia in this period. The normal 20 is nickel, and I see nothing strange about it.
