1978-D-Nickel:-The-Mint-Mark,-What-Is-This-And-What-Does-It-Mean?

On this image you see a D mint mark that looks "hollow" I don't know what other term to use.

What is it? I see it on a variety of coins, all years, this pic is just an example. Is it just Die Deterioration?

I think what you bare seeing is a filled Mint Mark which caused by a broken post then it has been hit, making what looks like a hole. Simply; the filled mm has been hit.
