Other then it being worn out, what do you suspect the error to be?

Not an error, it has just been repeatedly used in a slot machine. The action of feeding the coin into the pokie over and over wears off the reeding and the coins are constantly banging together so the coin also gets quite dinged up.

I agree. I see a ot of questions on these. Happens frequently to quarters as well.
