ALWAYS start from the assumption of PMD.

Only when you've eliminated that and can provide an explanation of how what you are seeing occurred during the minting process, can you jump to an error.

A side hit explains the metal movement.

Otherwise, you are stringing together a tiny Cud, and something in the coining chamber that pushed metal away from the collar.
