Hello. Has anyone ever seen something like this? I'm not sure if it's an actual error or some type of post damage. I can't think of what could cause these. Thank you in advance.

Looks like PMD. I see pushed metal and each gouge looks the same, caused by the same tool.

Please excuse my ignorance, but what is PMD? Is part of the process? I don't know anything but about this, but I'm interested in learning. I'm assuming it stands for post mint damage? What kind of tool or holder could cause that?

Straight from the CCF Numismatic Glossary: PMDShort for post mint damage. Damage sustained after the coin has left the mint.

The reverse might reveal a little more about the specific cause of damage, but as others have said, this is clearly post mint damage. The coin didn't leave the mint like that.
Snips could do that. Maybe pliers. I almost wonder if the coin was in a bezel or mount, but who would put a 1995 quarter in a bezel?

I agree with PMD/PSD (Post Mint Damage/Post Strike Damage). My vote is for cable cutters which resemble a parrot's beak in shape. 
Words of encouragement are one of the major food groups. We need to consume them regularly to thrive and grow.
