I agree with damage but there may be a die chip between the handle of the sword and the finger above it. Hard to tell from the photo.

Best to include photos of both side along with the rim....I have a feeling this may have been a Dryer Coin, due to the "mushy" look of the devices. Along with PMD of the flattened area.
