Looks like the mint mark took a hit, shows Machine Doubling, or Die Deterioration Doubling.

Was just reading on wexlers about 1988 D and there was a damaged punch used that looks a little like an RPM . But would agree that this coin took a hit . I could not find any listing for 88 D on any site.
