1976-D Bicentennial Quarter With Folded Lamination?

Snagged off ebay for $2.50 and free shipping, so I figured why not. The A in Dollar is only partly formed. A large flake of copper stands over the surface. I assume this is a lamination or a problem from fusing the clad layers. Maybe not? I'm not entirely sure what specific error this is. It seems like loss after strike is a possibility, since the A has missing bits. I suppose another possibility is a partial retained strike through of a copper flake contaminant.

I ran across this bicentennial error quarter on ebay, and it immediately reminded me of this coin. It's certified as an NGC MS65 error (6485304-010), reverse struck in copper fragments. While the copper fragments on the coin face are completely struck into the cupronickel and fused, the fragment on the coin edge is a bit ragged, just like my coin.
