2001 P North Carolina Quarter - Improperly Annealed Or Damage?

So I found these coins a while ago, and I'm curious if they are improperly annealed or if they are just damaged.. One is a 2016-P Harper's Ferry quarter, then a 2001_P North Carolina, and last is 2013-P Jefferson nickel... Any information would be great! Thank you!!
