This was pulled from a mint set. Could this be caused by a machine in the packaging process or something else?

I thought maybe die scrapes, but those appear to be raised. https://www.error-ref.com/die-scrapes/

This appears from time to time.
It is caused by the crimping machine when the sets are
assembled. Sometimes the coin is too close to the edge
and gets caught.

yep, nice catch of this packaging damage, sometimes we see that more of a coin that got nailed by the heat crimping machine.

Post strike damage, but not post mint damage since the crimper got it.
