1979 D Quarter: Help Identifying Errors

It's essential to have it in the title. * I believe I have a multiple error 1979 D quarter. The mint mark is filled in. Doubling on both obverse and reverse. The planchette is damaged on obverse and it's slightly off center strike 3%? Are my observations correct? Any help is greatly appreciated thanks for your input and time.
