Quote: The error formed when three planchets of different denominations entered the striking chamber at the same time. Should read: This mint sport was created when a worker placed three planchets of different denominations entered the striking chamber at the same time.
Inordinately fascinated by bits of metal with strange markings and figures

Intentionally created or not, still a very cool and interesting piece.

That's one heck of a deliberate defect.

Wonder if the mint workers get fired if caught doing these kind of things?

They shouldn't get fired, just cosider all the excitement and ooooe's and awwwwe's the numismatic community gets because of these assisted errors and on top if it they always carry a hefty premium

The image shown by @Chase007 is confusing to my eye - makes it appear that there are 6 parts to this assisted error.
I appreciate the link to the article which clearly shows it is 3 separate planchets that were struck at the same time.
I am not a fan of employee assisted mint errors. But they do seem to be accepted in the marketplace.
Take a look at my other hobby ... http://www.jk-dk.art
