Machine Double on the date is understandable but on the mint mark also ? I thought it was added after the fact ?

The mintmark is added after the fact to the die. The coin is struck after that, and MD is random, random, random. 1969 in particular (at least in my short career) runs rampant with MD. Some of my best MD examples are '69-S.

Just to be clear here....

1. Die is hubbed.
2. Mintmark added to die.
3. Coins struck.

Machine Doubling happens during #3.
