What causes the stretch look? It looks like the letters balloon at the bottom and the dye stretches round the head and face.

Severe die erosion, that would be a VLDS(very late die state) coin. 1964 was the first year of mintages over 1 billion which was not exceeded again until 1999. 1964D still holds the top mintage after 46 years so you can imagine the extend to which those dies were overused.

Also the nickel in the planchets made the die wear more than other coins. Just heavy die flow from extended use. You see this when the fields and the devices flow together and flow over each other.
