Help Identifying My Error Quarter

Hello,
I was sorting through my silver quarters, and located an error quarter.
I searched and could not locate an exact name for what happened to mine.

It is a 1956 Washington quarter, that has some type of die or stamping error. Any input on the definition for this error would be greatly appreciated. My first guess is that it is slightly off center, but the lines on the back intersect the "UNITED STATES OF AMERICA". Leading me to believe it has two errors?

The error is: on the front it has a recessed area from approx the 2 o'clock to the 5 o'clock position (from the "Y" in liberty to just before the date)
On the back it starts at the 11:30 position and ends at the 4 o'clock position. (It is a set of lines that start at the "T" in States, and ends tapering to a point below the "A" in Dollar.)

Also all of the reeding is well defined and intact.

Looks like a coin that was hammered by another coin. not a mint error, just damaged.

Would that have been hammered between two coins, to create the damage on both sides?

Correct. I was just looking at a couple coins that someone damaged that way just a couple of minutes ago. If they used a vise to do the squashing they would be more uniform.
