Can anyone here give me a detailed explanation how this occurs?
What is the proper term for it?
Does it add any value to the coin? (Seems fairly common to me)

all I see is a scratched Kennedy with a slight MAD, what are you seeing?

I see a slight MAD (MisAligned Die). Quite common and no added value until it begins to cut into the design.

OK I have the MAD part but how does it happen I thought the collar held the coin in place.

Assuming the reverse looks normal, it is a misaligned die, and yes they are common as you said and do not have any additional value unless it cuts into the devices as chugly stated.

If the reverse is off in the same direction, it is just a minor off-center.

Occasionally, you will see the reverse misaligned in a different direction.

thats why the reverse is centered the observe strikes and if it dont hit the coin dead center you get a MAD

But if this is the case the Die would have to be smaller than the diameter of the collar. Otherwise the die would be damaged if it was misaligned. I don't wish to sound dumb but I have no knowledge of reeded edge coins.
So if I understand correctly the raised rim of the coin is formed by the metal between the die and the collar?

The obverse die (hammer)is adjustable. When the alighnment isn't correct, then the obverse can be affected. It will still have the reeding and the reverse is centered. Thus the term Miss-Aligned-Die.
