Your coin is DDO 5-O-V, FS-102. In a PCGS holder, graded Proof-65, the variety sells for around $120. It looks like your coin has some PVC damage, but that's easy enough to remove, if you're careful not to let the coin get hairlines.

The doubling on the date and designers initials is a place to start. (See image above.)
