It's a very dramatic-looking error. But there's no reason to suspect it was struck more than once. This is a full, centered, first-strike brockage/broadstrike that exhibits strong cupping. The top coin inevitably curls up toward the hammer die when it's struck. This is also true of the equivalent U.S. coins, which are also mis-labeled as die caps.
