I've run into quite a few of these multi-struck Malaysian coins in the 10 and 20 sen denominations. The number of strikes in my small stash varies from four to 22. I have no reason to think that any were intentionally fabricated, but I have little doubt that they had help getting out of the mint.
Error coin writer and researcher.
