Can anyone tell me what was the cause on this coin? I have seen coins with Machine Doubling, but I have only seen that effect on one side of the lettering. This seems to be on both sides.

It is Machine Doubling. The dies movement created the damage you see on those devices. It happens. Machine Doubling can even be different from coin to coin if you have an OBW roll you can find this true:
