Interesting how the rim looks still intact but just pushed in. How does that happen?

It is just simple contact displacement and not removal, more commonly seen on the dates and mintmarks of coins damaged by a counting/rolling machine where the digits and letters will appear malformed but intact. See this post for more images of contact displacement.

Usually if it looks like it could be damage - it is. Errors are generally easily distinguished from damage.
