I saw this liberty and the bottom of it looks like there are an extra set of letters or it was smashed on the press.

How would something like this happen ?

Machine damage can create that illusion. Sometimes flattening, pushing. But it came from a normal die with the machine moving after/before/bouncing during the strike.

If it were from hub doubling the devices would be showing extra width:
