I'm going to say they called it PR64, and given it's in an older holder it developed the ED after it was slabbed. Now a Details example. Edit: Ahhh macmercury beat me to it

It's a proof that has deteriorated over the years in its plastic coffin.
PF66 would not surprise me one bit!

I'll say MS details (ED). Awful looking coin.

PF-63, there are a handful dings and bumps on Miss Liberty.

I note that there are no plastic coin support claws around the diameter of the coin. That suggests to me that the coin pictured is not slabbed.
Was this coin pictured in this way before it was submitted to PCGS?

sel- It's in an older holder, possibly OGH, they didn't have the plastic coin support claws.

Head scratcher . This coin IMHO could not have been sent to PCGS in it's present condition . I'm at odds

I'd say PR-64 when it was encapsulated by PCGS 30 years ago. The ugly toning probably developed from some chemicals left on the surface.

I am going to take a wild guess at PF67 in an OGH. The mint most likely used some type of chemical that caused a lot of the 1941 proofs to develop ugly and strange toning.
