Just coin contact. Note the affected area is incuse. (Just like what coin contact would show) on a doubled earlobe, that area would have been raised, not incuse. The size of the earl lobe is totally normal. Not enlarged or showing spread. Just a contact mark.

As stated, just a contact/hit, the ear is the same size, other than light displacement, by the damage.
