I am thinking this might be some type of strike through. The thing that is throwing me off is the way it bulged the edge out - this makes me then think PMD? Your thoughts?

I'm thinking rather vicious post-strike damage of a blunt force nature.

Whenever you find a coin with sunk in damage on one side, and bulging at the same point on the other, 99% of the time it's damage.

I'm with Dearborn, the rim should not be damaged in such a way. PMD.
