D = damaged MM and a slightly tilted die strike, a bit of overpressure strike as well.

S = Damaged MM puncheon where part of the inside design has become chipped and passes this damage to the resultant punch.

Crazyb0 is correct. It's a Damaged Mint Mark punch issue.
