How can you tell PMD the neck appears blobbed and the reverse
- Debris in die ?

The coin was hit/struck with something on the Rev. and the coin is protruding from that hit on the Obv. It's a spender, spend it.

Spender ( PMD) for sure, however I've never seen Washington with a goiter

Possible damage from heating the coin with a torch, creating a bubble between the layers. Looks like the reverse bubble was flattened. Just a damaged coin.
