Can anyone verify if this might be a "D" stamped in error or am I imagining things.

I see a normal 1986 damage. I see a little die crack but not more then.

The D was hit. you has a die crack from the left leaf under the first leaf right you see of the tail feathers, the rest is OK.

That is not a D, it would be impossible for that to happen during the striking of the coin.
As mentioned, it is damage from taking a hit.
