This one may be a little hard to understand from these photos. The edge of the coin @ K-9 is raised above the obverse by 4 mm. (Think of the shape of a ballcap visor.) The coin conforms to the shape of the obverse die as if it were intended to be molded onto it. I'll get it back out and try to capture an image that shows the shape looking at the edge if anyone would like to see more. An interesting error. But it seems to me that such obvious mistakes cannot be missed. One of the employees of the mint should have noticed it and destroyed it. I agree with those who say that this is all "custom-made".

This one could have been 'custom-made' because it was sold to a dealer along with several hundred other error coins. The person that sold it may of had some kind of tie to the mint as he did this about every 6 months or so for a number of years. He also could have a tie to one of the cash handling services (Loomis/Wells Fargo, GardaWorld, Brinks, etc.) that get bulk shipments from the mints and roll the coins before they are sent to the banks. nss-52 said... This may help show what I'm saying about the shape. If you were to place this coin on an older Jefferson design obverse die, it would fit perfectly with no gaps across the face, rise and taper of the die.

This is an off-center strike with a stiff collar error. You could alternatively describe it as an off-center strike with a very strong collar scar. In either case, the stubborn collar resisted depression and instead forced the unstruck crescent upward.
Error coin writer and researcher.

@mikediamond Thanks, as always! I'd read an article where you talked about a stiff collar error, but I didn't even think to associate it with this coin. Great to know what this error looks like!

Now I have a name for my ballcap-shaped off-center coin from early 3rd century Nicopolis ad Istrum...  ...except back then they probably didn't use collars, so it was probably a different situation. Same (or extremely similar) result, though.
