Considering the surface of one coin touching another coin while on its side, it would seem that a coin had to roll onto this one with some huge presure... PMD?

Most likely it happened outside of the mint, but maybe some kind of machinery or something did it there. looks kind of like a coin was driven into it, like with a hammer, not rolled across.

I think this is PMD. Also remember, PMD is anything that happens to the coin after it is struck at the coining chamber, even it is happens somewhere later at the mint.
