I would say PMD mainly because I have no idea how this could happen during the minting process.
Oh! by the way,can we see the rest of the coin (obverse and reverse)?

This coin brought to you by the letter "O" :-)

It's reverse to the "LIBERTY" in the upper-left-hand corner, so I'd wager it's incuse. With that in mind, it looks like someone was testing out a letter punch or a gromit setter.

PMD. If there is flattening opposite side, it was a punch, if no reverse damage, it was engraved.

@Jayman931 - The lighting's fooling you. It's like one of those "view a 2D cube on the side and watch it pop in and out" eye teasers. Compare the shading on the circle to the shading on the Y of LIBERTY and you'll see they're the reverse of one another (it's light on top edges of the Y but on the bottom edges of the O). Since the Y must be raised, the O must be the reverse. :-)

That's damage. One hint would be the fact that the coin is bent from the pressure of whatever tool struck it.
