Is this just a slight MAD with PMD? Seemed strange that the PMD would be just outside where the die was misaligned.

It kind of looks like it was almost a partial collar error, but something else happened to it.

Just damage, not an error involving the collar. The left side of the rim has been flattened, causing a bulge on the edge.
