I noticed this imperfection on one of my 1976-S (Silver) George Washington Bicentennial Quarters. The coin is graded PCGS PF69DCAM.
The first thing I thought of was that it looked liked a 'horn.' Similar to ones sometimes found on the forehead and back of the head on Roosevelt dimes. Any Ideas?
