Looks like a variation on John Croker's George I coronation medallion, with the legends repositioned. I believe the reverse reads INAVGVRAT XX OCT MDCCXIIII.
Found a reference that said there was a "poor man's" version of this medallion, which was cruder, and distributed among the Great Unwashed. This is probably one of them; smaller, so the obverse legend was probably shortened, and the reverse legend taken out of the exergue and put around the edge.
