During roll hunting I have noted this difference in the 1989 P MMs. In hand, The one on the left is a little larger looking than the one on the right (not sure if its optical illusion or not). I have found only three of the one on the left.

I know the one in the pic just looks like it has been mashed b/c of the hit on its bottom, but the other two of this variety are "clean."

Does this look legit enough to be considered an actual variety?

Die wear affects the size of the mint mark. The punching also makes some larger as well. Circulation contact. They are from the same punch. The one on the right is a fresher die than the one on the left.

Oh, so they use the same punch for all the MMS? interesting. I am surprised at how different the position of these two Ps are in relationship to JFK's neck.

The left one is much closer to the neck.

So if I would look closely at the strike details then the half on the left, overall, should have less crisp details?

The mint marks were applied by hand until 1990. So they could vary in strength, location and depth. Often if the mint mark is the only thing affected by machine damage/ejection, it is because it is taller than the other devices. After 1990 they are part of the design of the master dies.

So how do designations of small and large MMs get added to the hobby as actual varieties? From what I gather this is not one - and that is fine with me, I am just wondering how things like this happen.

These are added most of the time the year they come out. They changed the punch during a portion of the year.

There was several examples found. There was not just one example.
1946 they used three different mint mark types:

The ball serif being the lowest number found. Again, not just one example, but several dies had this mint mark.
the 1941 year also had the large and small mint mark.

The years that this happened, most of the time the same mint mark punches were used on several denominations that year. Each denomination has its common and rare occurrences on the dies. But again this in not on a single die.

The dies reported are the new/unlisted ones that need to be listed and not too minor to list. Usually doubled dies and RPMs.

They are from the same punch

Does this mean that there was truly just one punch laying around at the mint branch or that all punches had the same design?
