Found this while looking through my great grandparents Mercury dimes today (they owned a laundromat in 30s and 40s)

Fascinating. I don't see much in the way of displaced metal; I'm honestly thinking that is as it was struck due to a planchet defect.
Rest in Peace

I think this may be a defective planchet although I can't say for sure.

That's part of my thinking - it reached the upsetting mill that way, and enough metal remained for a reasonably-whole rim to be upset.

I think it's a strike-through.
A piece of foreign debris was on the coin when it got struck.
After the coin was ejected from the striking chamber the extra piece fell off leaving that mark.
With a bit of work you might even be able to determine if the piece was from another coin.

If I'm right it will have some premium but to a narrow market.

The other side appears to be normal.

I don't think it's a strike through. I think it's a defective planchet before it went through the rim upset mill. Nice find, with a small premium I would think.

I also wondered if it was a strike-through? I could be totally wrong though.
Perhaps some hard gunk fell out of the reverse die and was struck into the obverse?

The rim prevents me from thinking strikethrough. I don't see that happening with no rim involvement.

Thinking further, this must have occured before the upsetting mill.
