IF the feature on the obverse lines up with the feature over "Pluribus" on the reverse, then at the least it is a rotated die. I'm at a loss to explain how something that could "push" part of the rim inboard on the reverse; could do so without damaging the reverse. A split planchet might explain it. Just not sure.

It looks like you can see the corresponding ding just below his shoulder on the obv.
