I will bet it is graded 63 and turned in the holder. It deserves a details grade.

Yes, that's spot toning, more iridescent than is evident in the photos, one on each side. PCGS typically doesn't seem to take much account of such.

I don't know, the spots look like some kind of dip residue or acid treatment that wasn't rinsed off properly to my eye. I'd consider it damaged and wouldn't purchase it, but to each their own.

It reminds me of a coin stored in a cardboard 2x2 that had pinholes in the mylar window.
