OK, here's what I can offer:

How are PNCs put together?
The "putting together" method can vary by manufacturer, but the coin "card" (with embedded coin) is typically inserted into a custom-cut envelope. Each component is produced separately and then combined.
Quote:
Is the coin enclosure produced on the printer or is it inserted afterwards?
The coin/medal card is produced/printed separately and then typically inserted (with embedded coin) into a pre-printed envelope.
Quote:
How does the coin get into the enclosure?
Typically, a coin/medal is placed in/on the enclosure card and then attached/sealed to it.
Quote:
Does it come in two halves, coin popped in, enclosure sealed with heat?
Or is the enclosure sealed around the coin?
I'm not sure exactly what is being asked here...but the process goes something like: 1) A PNC's insert card/coin card is printed/produced, 2) A coin/medal is inserted/placed on it (sometimes in raw form, sometimes in an enclosure - flip, capsule, pliofilm pocket) and 3) the card is folded and/or sealed around the coin/medal.
Quote:
Are there any production figures for PNCs?
I'm not aware of a single source for such information, but some producers advertise production limits. Final sales totals are often less available, however., especially from third-party manufacturers vs. national government mints.

When were PNCs first issued?
I don't know an exact date for the first PNC - maybe another CCFer does - but I know that some were produced as far back as the 1960s.
