I've seen this before on other coins before. I am just not sure what it is cause by exactly. Any thoughts?

It might be a die gouge if it is raised. If incuse, then damage.

I believe I've seen something like this before on here and the consesus was that the die was dropped before being installed.

But the narrow size of the OP's mark on his coin made me think it was a gouge. A dent would probably be larger than that.
